Álvarez started his club career in 2005 in his native Argentina. He played there for the youth teams of Boca Juniors. Álvarez played there for a year when he was snatched away by CA Vélez Sársfield and played there two years in youth before moving on to the first team.
Álvarez played his first game at the highest level in 2008. In 2010, the midfielder broke through during the Copa Libertadores. He started out as a substitute, but grew into a basic player. He scored his first goal against River Plate, which won the match 1-2. Álvarez became champion with Sarsfield and made it to the semi-final of the tournament.
Álvarez was transferred to Inter in 2011 for an amount of 11.7 million euros. His debut in the first team of Internazionale was in the Supercoppa, against AC Milan. He scored his first goal for Inter on November 22, 2011, against Trabzonspor, after a pass from his fellow player Diego Milito.
